[Pdns-users] "Unable to insert a record of type #0" error causing PDns to no longer work

We recently got our PDns/Win 2.9.15 server up and running.  From time to
time, the log shows the following:

Tue 27 Sep 12:38:38 (2): Unable to insert a record of type #0 for 'home.net'
... or other domain names.

Immediately following this, the server then can no longer resolve *any*
names.  All requests following this are rejected:

Tue 27 Sep 12:38:38 (2): Not authoritative for 'www.computersynth.com',
sending servfail to 184.73.105.137
Tue 27 Sep 12:38:38 (2): Not authoritative for 'Home.net', sending servfail
to 74.245.120.116 (recursion was desired)

Has anyone else seen this?  How is an add record request coming into the
system from the outside and is there a setting to block this?
